Health
Care:
TERI, Inc. values the health and
wellness of you and your family and is able to continue to offer
rich health plans to eligible employees. Health care insurance is
offered through Kaiser Permanente and Pacificare at a minimal
monthly cost for single coverage. Both plans offer employees a
choice of a high or low option HMO plan. Employees may buy up to
the high option plan of either health care provider for an
additional cost. Regular full-time employees are eligible to enroll
in one of these HMO plans effective the first day of the month
following the completion of the 90-day initiation period. Employees
will be contacted to attend a benefits meeting at time of
eligibility. Dependent coverage is available under both plans and
is paid for by the employee. Employees may elect to use pre-tax
dollars to pay for health care premiums.
Dental
Care: Dental insurance
is offered through Golden West Dental. There are two plans
available: Prepaid Plan and True Advantage PPO. Regular part-time
employees working 20+ hours per week are eligible to enroll
in the Prepaid Dental Plan and regular full-time employees are
eligible to enroll in either of the two plans effective the first
day of the month following the completion of the 90-day initiation
period. Dental coverage is a voluntary employee paid benefit.
Employees may elect to use pre-tax dollars to pay for dental care
premiums.
Group
Life/Accidental Death & Dismemberment Insurance:
Group Life/Accidental Death & Dismemberment Insurance is offered
through CIGNA Group Insurance. Regular full-time employees are
eligible to enroll in this plan effective the first day of the month
following the completion of the 90-day initiation period. Employee
coverage is paid for 100% by TERI, Inc. There is no option to add
dependent coverage to this plan. Employees receive a $10,000
policy.
Supplemental Term Life Insurance:
20 year Term Life Insurance policies are offered through American
Heritage Life Insurance Company. Policies are available to
employees and their spouse in the amount of $25,000, $50,000, or
$100,000. Regular part-time employees working 25+ hours
per week and regular full-time employees are eligible to
participate effective the first day of the month following the
completion of the 90-day initiation period. Premiums are based on
age and benefit amount requested. Supplemental Term Life Insurance
coverage is a voluntary employee paid benefit.
Supplemental Accident/Sickness Insurance:
Policies are offered through American Heritage Life Insurance
Company/New Liberty Insurance. These plans provide income
replacement that will pay over and above state disability insurance
(and any other insurance currently in force) for employees that are
totally disabled due to sickness or accidental injury. This plan
will allow employees to better plan for elective surgeries,
maternity/pregnancy disability leave, or provide assistance at times
of unexpected injury/sickness. Regular part-time employees working
25+ hours per week and regular full-time employees are
eligible to participate effective the first day of the month
following the completion of the 90-day initiation period. Employees
must earn a minimum of $1000 (gross earnings) per month to be
eligible to participate. Premiums are based on the level of benefit
requested.
Flexible
Spending Account:
Full time employees are eligible
to participate in the Flexible Spending Account effective the first
day of the month following completion of the 90 day initiation
period. The Flexible Spending Account is an
employer-sponsored benefit plan that allows employees to have money
deducted from their pay on a pre-tax basis to be used for
reimbursement for qualified health and dependent care expenses
(i.e., over-the-counter and prescription medications, doctors’ visit
co-pays, chiropractor fees, eye glasses, dental expenses, laser eye
surgery, etc.). Participating in a Flexible Spending Account via
payroll deduction may also reduce your tax burden and increase your
take home pay. By participating in the fund, you have the
opportunity to budget for health and dependent care expenses by
withholding a small amount from each paycheck.
403(b)
Tax-Deferred Retirement Savings Plan:
The 403(b) account is offered through Mutual of America
Investments. All employees are eligible to participate in this plan
at time of hire. A 403(b) is available only to non-profit agencies
and is the non-profit equivalent of a 401(k). Employees under age
50 may contribute up to $15,500 (tax-deferred) per year and
employees over age 50 may contribute up to $20,500 (tax deferred)
per year.
Educational
Assistance:
TERI recognizes that the skills and
knowledge of its employees are important to the success of the
organization. TERI supports self-development and educational
efforts by providing reimbursements for many of the expenses
associated with continuing education courses. All full-time,
regular employees are eligible for benefits under this policy after
completing 6 months of service. Employees may pursue a degree or
take individual courses at approved and accredited educational
institutions under this program, provided the course of study is
related to the employee’s current position or a probable future
assignment and TERI approves the course(s). Employees may receive
educational assistance for (1) class per semester/school session and
approval must be obtained prior to commencement of each course per
semester. An Educational Assistance Request Form must be submitted
to the employee’s Department Director for approval. Forms are
available from the Human Resources Department.
Ted
Williams Scholarship Fund:
TERI, Inc. established the Ted Williams Scholarship Fund for the
purpose of providing grant monies to TERI, Inc. direct care staff
for use in pursuing educational goals. Monies placed into the Ted
Williams Scholarship Fund represent donations from TERI, Inc.
Administrative staff and other donations specifically designated for
this fund. All TERI, Inc. direct care staff may apply for monies
from this scholarship fund after the completion of the 90-day
initiation period. Applications are accepted annually during a
designated timeframe. Employees exhibiting poor job performance, on
a leave of absence, or not actively working for TERI, Inc. are not
eligible.
Paid
Holidays: New Year’s
Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day,
and Christmas Day.
Paid Time
Off: Regular
full-time employees are eligible to receive paid time off (PTO) the
first full pay period following the completion of the 90-day
initiation period. Employees accrue PTO at a rate determined by
number of hours worked and length of service. For example, accruals
based on a 40-hour work-week are as follows: 4 months-1 year of
service=114 PTO hours (14.25 days)/year, 1-3 years of service=152
PTO hours (19 days)/year, 3+ years of service=192 PTO hours (24
days)/year.
TERI, Inc.
Corporate Fitness Center:
Employees are invited to pursue personal health and wellness by
joining co-workers at the TERI Corporate Fitness Center, located at
251 Airport Road, Oceanside. TERI’s Corporate Fitness Director is a
Certified Strength Training Specialist and Nutritionist. Group
exercise sessions, such as Yoga and circuit strength training, are
held several times weekly. TERI provides corporate fitness center
benefits at no cost to employees. All employees are eligible to
participate from date of hire, following an initial assessment with
the Corporate Fitness Director.
Chiropractic Care:
All employees are eligible from
date of hire to schedule an appointment for Chiropractic
treatments with Dr.Anthony Hagner, D.C., C.S.C.S. in the Fitness
Center every Thursday. The fee is $20 per visit, payable though
payroll deduction. Employees should contact Terri Williams by

Employee
Assistance Fund: The
Employee Assistance Fund assists staff with a legitimate life
situation that, with assistance, can be rectified. The fund
receives grant monies through donations from the agency’s management
staff. Employees in need of assistance may request a grant from the
Employee Assistance Fund by completing the “Employee Assistance
Fund” form. This form can be obtained from the employee’s
department director or the Human Resources Department. Employees
exhibiting poor job performance, on a leave of absence, or not
actively working for TERI are not eligible.
